A Personalized Melanoma Shot Has Cleared Phase 3, but the Crucial Numbers Are Still Missing
Moderna and Merck say their tailored mRNA therapy with Keytruda delayed melanoma recurrence in a pivotal trial, a breakthrough for cancer vaccines. Scientists welcome the result but want full survival, safety and subgroup data before judging its real-world impact.
A personalized mRNA treatment for melanoma has crossed the clinical milestone cancer researchers have been chasing for years. But the announcement’s biggest promise is paired with its biggest caveat: the detailed results have not yet been released.
The market reacted quickly, with one report declaring Moderna shares had doubled on the skin-cancer vaccine success.1 The more consequential development came Wednesday, when Moderna and Merck said their individualized treatment, intismeran, succeeded in a Phase 3 trial when combined with Merck’s Keytruda.
The study enrolled 1,137 patients whose stage IIB–IV melanoma had been surgically removed. In a double-blind, placebo-controlled design, patients were assigned two-to-one to receive the tailored mRNA treatment plus Keytruda or Keytruda alone for roughly a year. The companies said the combination significantly extended both recurrence-free survival and distant metastasis-free survival, without yet publishing the magnitude of the benefit.2
The treatment is built from up to 34 mutations unique to each patient’s tumor, aiming to train the immune system to recognize cancer-specific targets. Moderna CEO Stéphane Bancel called it “a pivotal moment for the field of cancer research,” while trial leader Georgina Long described a potential new treatment paradigm.2
That optimism is grounded in an earlier five-year Phase 2 readout, which reported a 49% reduction in the risk of recurrence or death and a 59% reduction in distant metastasis or death. Yet outside expert Lennard Lee stressed that the Phase 3 announcement still lacks the data needed to show which patients benefit most, how quality of life is affected and whether overall survival improves.2
The distinction also matters beyond the trial: these are therapeutic cancer treatments, not preventive shots such as the HPV vaccine. Yann LeCun amplified that framing, saying the new generation “treat cancer you already have” and that 2026 is when they “stopped being sci-fi.”
